[Frankfurt, Germany: A woman flees from robed pursuers. She is caught and stabbed. She says "From beneath it devours"] Dawn wakes Buffy from her nightmare. Buffy tells Dawn there are more people like the girl and they are going to die. A creature is burrowing beneath the streets of Sunnydale. Spike is in his basement room talking to a rat when the room starts shaking.
Dawn is exited about Buffy's new job - as long as Buffy does not talk to Dawn's friends. Buffy starts her first day of her new job and goes to the basement to find Spike.
Willow is packed to leave England, but she is not certain that she is ready. Giles tells her that she is needed in Sunnydale and to trust herself.
A woman, Nancy, is walking her dog when it disappears in a hole in the pavement. She is almost pulled into the hole before something very large comes out of it. She runs, straight into Xander. At Buffy's house, Nancy tells Xander, Buffy and Dawn what happened. They agree to investigate.
Spike arrives to help. He is not exactly welcomed with open arms. Dawn and Xander are angry that Buffy did not tell them that she met Spike in the basement a week ago. Spike and Buffy talk. Buffy, reluctantly, accepts Spike's offer of help.
Dawn tells Spike that if he hurts Buffy again she will burn him alive, literally.
Buffy and Spike go on patrol while Xander takes Nancy home. Buffy and Spike talk while he is examining the hole. She accepts that Spike has changed, but she doesn't know how and he doesn't tell her. The hole is back-filled with earth and the creature has gone.
Xander has taken Nancy home and they are talking. They are, awkwardly, saying goodbye after she has asked him out when the creature burrows through the floor. Xander is suspicious that she was attacked twice in the same night. She mentions that her ex-boyfriend, Ronnie, would enjoy the situation and that she made a wish about him.
Nancy and the gang go to the Bronze to see Anya, who admits turning Ronnie into a worm demon. Anya refuses to reverse her spell. When Spike threatens her, to get her to reverse it, she sees that he has a soul again. To stop her telling anyone he hits her. He ends up fighting Buffy and taunting her, making her believe that he is his old self.
Buffy leaves to find Nancy, who left during the fighting. Nancy is chased by the worm. Xander tries to convince Anya to reverse the spell. Buffy saves Nancy from falling off a fire escape, but the worm burrows through the pavement.
Spike arrives and beats the worm with a metal bar. Just as he is about to stab the worm with it, Anya reverses the spell and he stabs Ronnie. Spike reverts to his incoherent state, but does tell Buffy that something much worse is coming. Then he runs away. Xander and Anya arrive in time to see the results of Anya's spell.
Buffy leaves to find Spike. She finds him in a church. He, eventually, tells her that he got his soul back for her and that he is re-living all the deaths he caused. He leans on a cross. He is tired and wants to rest.
